If you visit a sick person in the hospital every day for a week, but go home every day after the visit, that is 7 visits.
If you call on a family at the funeral home it is one visit; if you then attend the funeral on a different occasion, or day, it is another visit.
A blood donation consists of one pint.
Count the number of pints donated during the year and enter that number. Record zeros for any line for which you had no contribution of visits or hours.
We need a form from every member, no matter how small the number of visits or hours contributed. Please print the form, fill it out, and submit as soon as possible but not later than December 31, as this will help in consolidating potentially plus inputs by the mid-January before the deadline.
